To be eligible to use the Auxiliary Seat, you must watch the video and fill out the following form:

PASSENGER SAFETY BRIEFING AND EXIT SEAT PROGRAM VIDEO FORM

The form must be signed by your supervisor and sent to the Flight Department (instructions are found at the bottom of the form).

This must be done every 12 months.
